Welcome to the 2024 Fuel to Go & Play Club Championships Nomination Form.
Rather than having separate nomination forms for each age group, Hockey WA are combining all age groups into the one form. You are able to enter multiple responses if you are unsure how many teams your club/association will be entering.
Tournament dates for each age group are below:
J 7/8 Girls – 1 July 2024 – 3 July 2024
J 7/8 Boys – 3 July 2024 – 5 July 2024
J 11/12 – 2 July 2024 – 4 July 2024
J 9/10 Girls – 8 July 2024 – 10 July 2024
J 9/10 Boys – 10 July 2024 – 12 July 2024
The nomination fee per team is $750.00 (incl. GST). This is calculated at just under $50 per player if team is made up of 16 players. Any modified formats that arise from nominations will be attributed a fee that reflects a similar approach.
Championship Rules will be emailed to the team contact with the Championship fixtures. Every effort will be made to ensure that these do not change, however team withdrawals will determine this.
Please note that nominations will only be accepted from Metropolitan Clubs and Regional Associations financial with Hockey WA from 2023 as per Hockey WA Rules (or new Metropolitan Clubs or Regional Associations applying for membership in 2024). In entering teams in the 2024 Fuel to Go & Play Club Championships, a Metropolitan Club or Regional Association must agree to re-affiliate with Hockey WA for the 2024 financial year.
Nominations Close Friday 21st June 2024, 11:59pm.
Late nominations will not be accepted unless there is a vacancy.

Permit Player Request Form
If you are seeking special permission for a member of another Club/Association to play for your team, please complete a permit request form located here: https://www.hockeywa.org.au/surveys/5730/
Permit request forms must be submitted no later than Monday 24 June 2024.
Please note, if no form is received, then it will be assumed that all players are from your club/association. If this is proved false during the Championship, penalties will be applied.
Completion

Please note that our Finance Department will issue an invoice to your club for payment.
