To notify Hockey WA of a change in club uniform, please download the "Change of Uniform Template"and send the completed form to Hockey WA Competitions Lead, Graham Pilcher, at graham.pilcher@hockeywa.org.au

To download a copy of the "Change of Uniform Template"

  1. Click 'File' and scroll to "Create a Copy"
  2. Click "Download a Copy" - this will allow you to have your own copy of the Template Powerpoint which you can use to fill out with your information and send to Hockey WA.
