Purpose of This Declaration

To support athlete wellbeing, performance, and appropriate load management, we ask all athletes to complete the following training and loading declaration. The information provided helps coaches and performance staff understand current training demands, identify any potential risk areas, and make informed decisions throughout the program.

This is not a selection tool, nor is it intended to limit athlete involvement. Rather, it is a proactive measure to ensure training expectations are aligned, athletes are adequately supported, and workloads remain appropriate leading into and during the competitive period.

We ask that this form is completed honestly and accurately, as this information plays an important role in maintaining a safe and effective State environment for all athletes.

Sample; Session 1 = 90mins, Session 2 = 45mins etc.etc.
