PLAYING SHIRT ORDER FORM

Please be advised that it is part of National Championships regulations that all teams have an alternative strip. Therefore all travelling athletes must have both a HWA Gold and Black playing shirts.

The current State shirt is the Indigenous Design, with a Surname and Playing Number. 

Players do have have permanent ownership of a specific playing number. While we make every effort to accomodate previously used numbers, there are occassions where number clashes occur. Especially if you have choosen a number between 1-16. On these occassions HWA will reassign numbers, using an allocation framework.

Completing the survey is a complusory requirement for all athletes in a named extended squad. HWA will only order the shirts in the quantity you specify. If you select 0, we will NOT order a shirt for you.

Due to the lead in period for Playing Top orders, Training Partners are required to order a minimum of 1 of each colour, as team replacements can occur right up until departure.

Please ensure you submit the correct qaunitites, size and within the deadline of this survey, as late orders can not be accomodated. There will always be laundry facilities available for teams when travelling, so at a minimum, 1 of each colour is all that is required.

As part of your selection in the team, you are required to wear the disignated state team uniform. To enable the team's apparel provider to arrange your team uniform in your size and for you to be provided with opportunities to purchase other non-essential items of team apparel, it is necessary for the apparel provider to be able to communicate with you. To that extent, we require to share your mobile phone number and email address, and for your consent for Hockey WA to provide these details to the apparel provider. The apparel provider will only use these details for the purposes of contacting you in relation to the team uniform and other hockey apparel and equipment.

If for any reason you do not wish for HWA to share your phone number and/or email address to the apparel provider, alternate arrangements will be made. Please advise if you are not prepared to provide your phone number and/or email address to the apparel provider so that alternate arrangements can be put in place.

All expenses will be the responsibility of the player.
